3. Important Health and Medical Disclaimers
The information on this Website is for general informational and educational purposes only. It is not intended as, and must not be taken as, medical, chiropractic, or professional healthcare advice, diagnosis, or treatment of any kind.
-
Nothing on this Website creates a physician-patient, chiropractor-patient, or professional healthcare relationship. Such a relationship is only established through an in-person consultation, examination, and mutual agreement to provide care at our clinic.
-
Always consult a qualified healthcare provider for any questions about your health, medical conditions, or treatment options. Never disregard professional medical advice or delay seeking it because of content on this Website.
-
Chiropractic and related therapies involve inherent risks and benefits. Individual results vary significantly based on many factors. We do not guarantee any specific outcome, cure, or level of improvement.
-
This Website is not for medical emergencies. If you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency facility.
-
Our services are provided within the scope of chiropractic practice under Colorado law. We do not claim to treat or cure all conditions.
4. Memberships, Wellness Programs, and Recurring Services
We offer membership and recurring care programs designed for ongoing pain relief, wellness, and maintenance care. Enrollment in any membership or recurring program is governed by a separate Membership Agreement or Patient Care Agreement (the “Membership Agreement”) that you will receive and acknowledge at the time of enrollment (in-office or electronically).
These specific agreements detail:
-
Services and visit allowances included in each tier
-
Recurring billing amounts and schedule
-
Auto-renewal terms
-
Cancellation and notice requirements
-
What is and is not covered
-
Any minimum commitment periods (if applicable)
By enrolling in a membership, you agree to the terms of the applicable Membership Agreement in addition to these Website Terms. We encourage you to read the full Membership Agreement carefully before enrolling. Pricing, plan details, and benefits are subject to change and are confirmed at the time of enrollment or upon request.
5. Payments, Billing, and Automatic Renewal
Payments for services, the new patient special, memberships, or other programs may be processed through the Website, our online booking system, or in-clinic.
For recurring memberships and subscriptions:
-
You authorize us or our authorized payment processor to charge your designated payment method on a recurring basis according to the plan you select.
-
We provide clear and conspicuous disclosure of all material terms (including renewal, total charges, and cancellation) at the time of enrollment, consistent with Colorado law.
-
You are responsible for keeping your payment information current and accurate.
-
Failed or declined payments may result in suspension of membership benefits until payment is successfully processed.
Specific billing cycles, amounts, failed payment policies, and late fees (if any) are set forth in your Membership Agreement.
6. Cancellation, Refunds, and Termination
Cancellation policies for memberships and recurring services are detailed in your specific Membership Agreement. In general:
-
Cancellations typically take effect at the end of the current billing period upon proper notice.
-
Where enrollment occurred online, we aim to provide a straightforward online cancellation method in accordance with Colorado requirements.
-
We may offer a retention or “save” offer during the cancellation process, but you will always have a clear path to complete cancellation.
Appointment cancellations: Please provide at least 24 hours’ notice when possible. Late cancellations or no-shows may be subject to a fee as disclosed at booking.
Refunds are generally not provided for services already rendered or for past billing periods of memberships. Promotional offers (such as the new patient special) have their own terms. Contact us promptly with any billing concerns.
We reserve the right to suspend or terminate access to memberships or services for non-payment, violation of these Terms or the Membership Agreement, or other good cause.
7. Online Booking, User Accounts, and Communications
Our online booking is currently facilitated through a third-party platform (such as Jane or similar). By using online booking, you agree to any applicable terms of that third-party provider in addition to these Terms.
You are responsible for providing accurate information when booking or creating an account and for maintaining the confidentiality of any login credentials.
We may send appointment reminders, confirmations, billing notices, or other service-related communications via email, text message (SMS), or phone. Message and data rates may apply from your carrier. You may opt out of text communications by replying STOP (or following instructions provided). Consent to communications is not a condition of purchasing services.
